Honestly, no movie creeped me out like The Grudge and it's not even that good of a movie.
And Michael Myers' mask and movements combined with that music still get to me as well.

Jaws is an excellent choice. The movie that scared me the most was The Exorcist.

The Changeling (1980) also scared the hell out of me.
Cujo was also terrifying, I think that gets third place.
Jaws terrified me as a kid. Great movie. As an adult I think the Descent was scary as hell. It's also one of my favourite horror movies.
shareNot a fan of heights, close spaces,suffocation or Sandra bullock...outer space is my bugaboo...gravity kind of fucked with me for these reasons...but only on the big screen...seeing a three story version of sandy in her skivies...terrifying!
