If this was real Star Trek, she would have been forced to lose weight while at the Academy, or even been given drugs that would sped up her metabolism. Starfleet normally has standards on the fitness of their people. The only reason Kurtzman put her in his show is because he wanted to basically cast every loser reject he must have known in school, including the resident "fat chick," despite her being all wrong for Starfleet. In fact, Tilly isn't just a terrible candidate for working on a starship because of her weight; she's got serious anxiety issues, something you can't afford to have when living in space on a ship that might run into trouble during the 5-year mission.

Pretty much, Tilly either would have failed the application process for getting into Starfleet, or she would have washed out after a few months in there, or in the unlikely event she passed the final exams, she would have been given a permanent ground assignment on earth, rather than risk putting her out in space.

Weird how after centuries of scientific and medical advances there are still obese individuals.
