According to the first line of the Wikipedia entry on nursery rhymes:

"A nursery rhyme is a traditional poem or song for children in Britain and many other countries, but usage of the term dates only from the late 18th/early 19th century."

I think it's saying it's a poem OR song. One of the two. It's not saying that two are equated.

Pretty sure it's always been a song, though. But definitely not a nursery rhyme AND a poem.

That may be. Are song lyrics automatically poems if they meet certain criteria?

I really have no idea. I know almost nothing about poetry.
