He says "And she ain't even the worst thing that's in there" (no plural). Like @acelesson I assumed he was talking about the man, since he knew about all the kidnapping, raping, offspring,... that had been happening in that house (which was way worse that the woman herself, since she was somehow another of his victims). Probably he didn't see all of that himself but stories were told (and once you saw the woman around the neighborhood at night you could trust the rest of stories).

What stories??? We never heard anything about any stories... how would anybody know what was going on in that house/basement? Sure there is apparently a crazy freakishly woman running around... but what does that really explain? And if someone knew why didn't they try to stop it...

It seemed like only the homeless knew.
If I was an homeless old guy, and saw that homicidal creepy creature run around, presumably attacking/hunting (she's got to eat), I would want nothing to do with it. Especially if I had witness my homeless colleagues get picked one by one. That's how I interpreted it anyway.

That is kind of my point... how did he know... he didn't seem very interested in exploring that house/basement... and if there were stories being told... then more than him knew... So it kind of doesn't make sense... but I guess you just have to accept these kind of inconsistencies in most horror movies...

There weren't legions of homeless people either. It looked like the neighborhood was dried up. Emptied of anything usable. Maybe he's like the last one (or one of the very few) out there and as an old timer he knew the backstory/urban legend since way back then or learned it through the years and then saw weird things with his own eyes. Cementing the whole thing.
To be honest, I have a bigger issue with how this little house kept being so clean and well maintained and preserved when only a cleaning lady (or crew, I don't remember) would only go when tenants were about to rent. In a neighborhood as sordid as this where even the cops don't want to go... it seems farfetched. More than the homeless dude knowing about messed up things that happened and are still happening in his town. To me at least..
