I think the scariest moment for me has to be the beginning of Candyman 2, in the toilets of the pub. When the door's swinging and you see a flash of him appear, and then when the character that gets killed turns around and looks in the mirror, he's behind him. I think this was one of the first horror movies I saw and it scared the crap out of me! I wouldn't look in the mirror for about a month!

Are we talking about moments that actually affected us longterm, or that made us jump, or that merely showed somebody menaced or killed? For the former, I would only have one or two moments that truly unsettled me:
The Ring - Samara crawling out of the well The Exorcist - Spider-walking scene (in the restored version) Creepshow - Ed Harris getting crushed by the tombstone, which seems to move of its own accord, and the dead guy crawling out of the ground The Shining - The beautiful woman suddenly turning old and horrible
Alice Sweet Alice - In one of the beginning scenes when Alice removes her mask to reveal a creepy mask underneath. <- even though it was just a mask that scene gets me every time.
The Descent - When the crawlers first attack.
Creepshow - Crate Monster.
The Vanishing? - When the guy got buried alive.
Suspiria - When her dead friend attacked her.
28 Weeks Later - When the man brutally killed his wife.
The Shining - Seeing Jack in the picture. <- not scary but it creeps me out.
In the last few years I can say that only one movie scene has truly left me terrified and that's the elevator scene in the original version of "The Eye". I still get nervous when I'm in one alone.

I remember seeing Blair Witch in the theaters and at the end when the camera first panned across the house you could just hear the whole audience tense up. It was awesome.
Even though movies don't get any scarier than The Exorcist and The Texas Chainsaw Massacre, I am mostly going to list movies that haven't been covered much (or at all) on this thread just to keep things interesting.
OPEN WATER- the whole movie is absolutely terrifying, but the nighttime scene actually made me teary eyed because it was so intense, and I've been in love with the horror genre since I was a little kid!
1408- Wow, I am shocked that nobody has mentioned this one yet. This movie has countless scary moments, but the one that got me the most takes place on the ledge of a building.
ERASERHEAD- The dinner scene. I was very happy to see many other lists with Mulholland Drive and Twin Peaks mentioned, but I think Eraserhead is still Lynch's freakiest film.
DEEP BLUE SEA- What happens in the middle of Samuel L Jackson's inspirational speech. LOL!!!!
INVASION OF THE BODYSNATCHERS (1978)- The ending.
BLACK SABBATH- "A Drop of Water" is the scariest tale in any horror anthology.
THE FUNHOUSE- The final hour is one good scare after another.
WRONG TURN- when the group tries to escape the cabin while the psycho mutants are sleeeping.
And even though I don't have individual scenes chosen, I have to mention The Descent, The Signal, Pan's Labyrinth, In the Mouth of Madness, The People Under the Stairs, Scarecrows, and The Resurrected.
I haven't really seen a whole lot of horror films, but I have seen some creepy and scary movie moments...
Zombie - the eye gouging scene. To this day, I can't watch it.
Night of the Living Dead - the daughter eating her father scene. And the zombies in general. Just blank looks on people creep me out.
The Thing - as a kid, the scenes that took the cake for me were the defibrillation scene (not sure if that's how it's spelt), the blood test scene, and the kennel scene (when the dog transforms).
Alien 3 (i think) - when the alien is swimming after the survivors.
Predator - the predator's laugh at the end.
Misery - the hobbling scene. But it would have been worse if it stayed true to the book (Spoiler: she CUTS OFF one of his feet).
The Fly (1980's remake) - when they send the first baboon through the telepods.
Diary of the Dead - when they show the little girl's birthday party. That just creeped me out.
It - I saw It as a kid and any scene with It in it, just was too much for me. And I was one of those kids that wasn't bothered by clowns at the time. Think about that.
The Blob (1980's remake) - when The Blob "spits out" that little kid and he's already being digested.
I may have more, but I can't think of anymore right now...
Invasion of the Body Snatchers (1978)- The scene where Matthew is asleep and the pod people are forming. Also the devastating ending.
The Fly (1986)- Maggot baby dream
Gozu-the cow-head scene (at first it seemed like it would be really dumb, but it turned really spooky). The "milking" scene. The "birth" scene. This was the second strangest movie I've ever seen.
Eraserhead- THIS is the strangest movie I've ever seen. The part that scared me though was the sudden sight of the mutant baby being sick. The part where the radiator lady popped up and started singing "In Heaven, Everything is fine" was pretty freaky too.
Dark Water (the original)- The ghost girl and the mom in the elevator.
Ringu- a bloodshot eye...unnaturally rolled down...
Ju-On: The Grudge- the very last shot of the movie and THAT DARN SOUND EFFECT!
Pan's Labyrinth-The creature in the second mission waking up and getting its eyeballs in its hands.
i haven't seen many horror movies, or scary movies, but there have been some moments that have freaked me out (some aren't even really scary, just made me jump for a random reason)
Friday the 13th - the ending! well the last scene at the camp I should say...the scene after that isn't scary...but dude. Freaky. Friday the 13th - Alice waking up and saying Billy's name (only scared me because it was the first time I was watching the movie and i had my back turned to the screen for 10 seconds) Psycho - Ending. Norman. Bates. Jumping. Out. not even the skeleton really scared me, just Norman with a knife and a dress. Psycho - Arbogast's murder (my room is the first one in the hallway off the staircase so it adds the scary) Scream - RIGHT BEFORE THE CREDITS! ohmigod! I was watching the DVD one night alone in the living room and reading, and I'd forgotten about the final jump, and i jumped out of the recliner and onto the floor it scared me so badly. Prom Night (2008) - Medicine cabinet scene. The Shining - Danny's "creepy" voice as i call it. The Shining - random flashes... The Shining - The twins! OMG! THE TWINS ARE FREAKY. Halloween - Michael outside the high school window...that's creepy.
and basically any scene with a creepy face XD
and then an episode of CSI: sent chills up my spine once (scene at the end of the episode where it's shown that the minature killer is still out there)
