Was the diaper on face scene really necessary?

I was enjoying this movie then that disgusting scene takes place. The problem is, that with such disgusting scenes people want to forget the movie or not talk about it after leaving the theaters or recommend it to someone. That's why certain "realisms" are not included in movies. I certainly want to forget this.
