So the Joker slicing Gamble's cheek...How does an injury like that actually kill the guy?

When the Joker sets off the bomb in the MCU, he goes from having cops on either side of him/in front of him to everyone being down; presumably, they're all unconscious. Given where the explosion was positioned in relation to the Joker's standoff, how is this possible? Especially since he is still standing. Did his men invade the precinct and subdue them at that point?

I feel like these two issues weren't adequately addressed in the film. I still love it. I'm just wondering if anyone has the answers for this. Maybe it was in the novelization?

1. We don't actually see him cutting his cheek, for all we know he shoved the knife into his skull first.

2. He uses detective stephens as cover, and he doesn't die either. So we don't know if the other officers are dead or just knocked out.

In the second point I guess the police men were just stunned by the blast, the first point Gamble was supposed to survive and even appear on the sequels, they created his death scene in the editing room since they didn't actually shoot one.
