shareFrom what I read the issue with the Baldwin film was the armourer who got the job because she is the daughter of an actually experienced armourer. I would say she failed in her duties and then Baldwin failed to check the gun before firing it.
I don't see why actual guns should be used anyway. Surely they can make models of them and if needed act effects afterward to show smoke, muzzle blast etc afterwards.
That said, how many Westerns and War films have been made where not one person died due to being shot? I would say the fatality rate is a very low percentage. So again it comes down to mistakes and incompetence more than anything else.
