The bullies call him fatkid because they are bullies, but why do his best friend's call him that instead of his real name? Shitty friends.

Sometimes the best way to remove an insult is to turn it around on the people making fun of it. In this case his friends probably started calling him that because it lessened the effect of the people who were trying to insult him.
