Yea I think that is kind of a good thing. Being that this is a horror movie, that part gives it a real shocker moment. I was wondering just sense she was so obsessed with surgery but I guess she was more into just playing with the blood.

I disagree, while "playing with blood" is definitely one of her motives, I think the reason she was so obsessed is because she loved Grace and knew her sister was dying. Grace was the only person in Pauline's life that treated her kindly and showed unflinching love, and Pauline didn't want to lose that. If she only wanted to play in blood, she could easily have killed the neighbor and left her sister alone. She honestly thought she could save her, even in the end she thinks the surgery was a success.

Doesn't she say to his mom during the scene that she didn't know what to do with the body, so she practiced her incision on it? And i thing you saw her with her eyes open without any life.

I am pretty sure she is dead.

I'm actually surprised there wasn't some dream sequence or hallucination scene that showed the little sister "alive" - a blinking eye, twitching hand, or something. It would fit the genre.

I think the fact she opened her eyes with the blood coming out of her mouth was enough of a horror on it's own, it pretty much means she actually woke up moments before dying, that fact makes me feel sick to my stomach tbh.

I think she would have woken up right after Paulin started to cut her open.. Without proper anesthetic, the pain would have jolted her back in the consciousness.

Mmmm yeah it's a possibility, The shock and maybe even the chloroform still in her system would easily be enough to render her paralyzed from moving her body which is essentially just as horrifying as my first thoughts.

I was thinking she would die if they had different blood types, which led me to realize that there would have been a HUGE amount of blood loss that certainly would have killed her. But the bottom line is that HER LUNGS WERE REMOVED AND SHE LAID THERE ON THE TABLE for God knows how long, with no respirator to keep her breathing until the new lungs were transplanted. So yeah, she was a goner for sure. Heart-breaking final 30 seconds of the movie...

Just ended watching the movie. The ending defeats the purpose of the entire 'becoming a surgeon' dream. She literally killed her sister. I don't think her goal was to practice medicine, but to just play with blood...

Certainly she loves blood and slicing and dicing, but she took a lot of pains to very specifically attempt a transplant for her sister. I think she felt she could save her, however delusional that belief was.

What a lovely way to burn...

Why are people saying things like, "I do not think she could have survived". Pauline says to her crying mother that although the surgery itself went well, she was unable to keep her sister breathing when she disconnected the lungs (i.e. she was unsure how to keep the body living during the switch).

Do you really need a film to be so explicit that despite the fact that you can see what happens to the characters, and any uncertain is wrapped up with what is directly implied by the dialogue, now has to be overtly announced?

The only thing she says about the actual surgery is that she didn't know what to do with the body of the girl across the street so she practiced her incisions.

"Come closer. I want to show you the detailed work I did on Graces sutures. It's a mess... I know! It's.. it's just my first surgery! I haven't perfected my technique."
"What have you done...?"
"What have I done..? You mean, with the other one? I didn't know what to do with her body after I successfully retrieved the lung, so I practiced my incisions. You have to take a closer look mom. She's.. extraordinary!"

I think we all know Grace died during the surgery, I'm pretty sure we're just being hopeful she didn't because of how shocking the scene was, I cant see anyone not being shocked by that scene and wanting Grace to die to be honest and what's a world without hope? :P

Based on the dialogue and the lifeless blank stare of the sister on the table, I would think the transplant girls are both very dead.
