Did his step dad really sexually abuse”ing him, and was he really being raped at the hospital?

That's part of his delusional schizophrenia, and all these allegations were negated by his twin brother.

When Ralph warned Dominic to have his brother tested for HIV I think it was known then that he was being abused in that hospital.
