I was 10 was this came out and theaters and after that scene with the two kids getting killed, I had to leave since it freaked me out so much, since they didn't look much older than me. I agree that it's rare to see kids dying in a mainstream release, it's sort of one of the last taboos in movies.
I remember a kid being killed in Shocker (the main character's mom and siblings are slaughtered), Hostage (kid is shot in the opening scene), and Hobo with a Shotgun (kids in a school bus burned by a flamethrower and toddler burned by molotov cocktail).
I cheered inside when it happened. You're right, you don't see it on camera in a mainstream release and this one was done with some vigor. I got big respect for Del Toro after seeing it. Stayed true to horror in that moment.
