Steffie was definitely insinuating that Carlito was gay

First I believe she noticed how close Carlito and Klienfeld were; you could say they were like brothers. In the beginning of the film, in the club, Carlito shows his appreciation for his lawyer saving him yet again in an extremely intimate manner. People around them who don't understand their bond could easily mistake them for lovers and talk about them this way, and word could have gotten to Steffie. Later Steffie asks him why he doesn't have a girl and even asks him straight up why he never tries to sleep with her. I believe she truly thought he was gay and was trying to get him to admit it.

There are far worse things to be insinuated about. I think Mr Pacino is gay in real life. (I know: "seeing Bev D'Angelo ")

In the context of the film, it appears that is what was being insinuated, but then Pacino's character gets with Penelope Ann Miller, who's character and performance is the weak link. Pacino apparently was having an affair with her and insisted she be cast. This misguided, miscasting choice spoiled a part of his own movie and dropped it down a notch or two.
