The Ending

So they give the rich asshat the heart, and he dies anyway. But they don’t let us know any of that. Did he die on the table? OD afterwards?
That was a bit frustrating.

Also I don’t see how it’s ‘haunting’ them seven years later….how would choosing someone else change things?

Overall I did like it, I just thought the ending was a bit lacklustre.

" Did he die on the table? OD afterwards?"

You missed the conversation between Grammar's character and Stile's character in 2019(?) when they were walking along the beach. Grammar mentions that the son (Trip?) died of a self-inflicted OD six months after he received the heart.

"Also I don’t see how it’s ‘haunting’ them seven years later"

Yeah, unfortunately the clunky writing doesn't do a good job of illuminating the guilt that they live with later on.

Grammar's character fought against Trip getting the heart on medical grounds. Later though he relents when the hospital adminstrator convinces him that the greater good would be served if he voted for the worst candidate because they could do so much with the $25 million.

Flash forward, he feels guilty for his moral lapse. As self-punishment, he refuses to get his own transplant because he is now in the same position as Trip... medically it would just go to waste on him. Furthermore, Dan Hedaya's character is once again tempting him with a big load of cash to get the transplant and is willing to get the heart through shady means.

The earlier decision cost the relationship between Grammar and Styles who advocated for Trip on the basis his fiance was pregnant and that Trip would mature to meet his parenting expectations.

In the future, she now finds herself in the earlier position of Grammar... convincing him that despite the shady deal he has to make with Hedaya, the greater good would be served by him getting the transplant.

AFA Garofalo's administrator is threatened with exposure by the priest who was privy to the discussions and is aware that her decision was tainted. His silence though was bought and paid for too so he's not much better than any of them. This part of the story was weak and not developed very well.
