Holy CRAP

Where is everyone in here reviewing this amazing movie than seeing thousands on stupid movies that serve no purpose. It's a year old and there's NOTHING HERE.
Nobody cares. I recommend this highly. It was that great.

I'm not sure it's quite a year old. In some areas this film was released last fall, and in others, only this year.
Still, I agree, perhaps this is one of those good movies that just fly's under the radar. I rated it 8/10.
I do have one question: why did the nurse think something was wrong when she read the torn letter or note at the end?
How did she know something was wrong on the basis of that note and perhaps phone call?

Because the note revealed that Dr. Lawrence's mother or sister (I forget which) had had a stroke and had to attend to her, thereby leaving in a hurry.

And all the pieces matter (The Wire)

There was no reason for that note to be hidden. Other than for Michael to use it as a tool to distract everyone so that he could get at the chocolates which had nuts in them, which would be fatal if he ate them (as he wanted to die). When the nurse saw how straight-forward the note was, she figured out that Michael stole it for a reason...to create concern and uncertainty for the staff. Which distracted them enough for Michael to find a way to get what he wanted... 
