It is very clear that the boy pulls off the woman's nipple with a pair of pliers and the older sister sees this, but later the mom says that he was only feeling her body while masturbating (which we never saw.) The father asks if anyone was hurt and she mother says no. What the *beep* The woman had her *beep* nip pulled off.

There's most definitely a problem with the scene. Whether or not he was masturbating, he certainly was using the pliers to tear off the woman's nipple. That is clear when the sister covers her back up.
So the wife not saying anything about that is a great ommission, because the dad would have found out later (the next time he went to rape her) and he would have been pissed... The mom would not have liked the results if he had to find out on his own.
I thought that too, that surely the dad would have seen the damage done to her nipple the next time he raped her. I think there was a potential mistake in the plot at that point. When Peg hears the woman's screams and comes running, we briefly see the boy's hands holding the pliers several times, and each time both his hands are holding them; he couldn't have been masturbating, since both his hands were occupied with the gruesome torture. I can't work out why Peg lied to her mother about what she'd seen, there seems to be no benefit to the lie. Either way, the dad probably would have stuck up for his son, since he had also hurt the woman both sexually and physically.

I mean a minute or two later she did say she was leaving him. Or maybe he was masterbating and it didn't show it. She said something about "your rapist son" or something along those lines so he might of done something but it wasn't shown on screen.

Guys, the first thing we see before the pliers closing and twisting, which the boy did holding them with BOTH hands, was one hand jabbing her breast with them while the other wasn't visible. He must have alternated.
