Whose was the fourth face?

There is the main character Natalie Portman, her rival the newcomer, the retiree dancer who was crippled in a hit and run, but who was that creepy-looking fourth girl who appears during hallucinations, like when Portman was underwater in the bath?

I just rewatched this, and I'm pretty sure that's actually Portman's face in the bathtub scene as well. It does look slightly strange though, which I attribute to the lens being underwater combined with the creepy smile.
