Miller was repairing the machine firs t without the safety switch on. He asked for Trevor ' s help and then it went from there. Anyone could hit that switch accidently the way Trevor did.

I agree. And he even asked Miller if the safety was on and he said it was.

For that type of repair that machine should have been locked out and Trevor should have added his lock to the lock out.

That's what I was thinking during both accidents. Doesn't anybody lock out their machines anymore?
