I don't think I'm particularly stupid, but I didn't understand anything.
Could anybody explain it to me?

Well, I just love this kind of movies :) I enjoyed it but it got a bit confusing in the end :P About the kid, I think he was just caught while he was in the appartement trying to steal something, if his brother could break into it, so could is brother.. or at least that is what I think :)
The most confusing part to me is the relationship about the 2 female characters.. although I think the doctor was always a girl, if I recall correctly when they checked on who lived on the neighbour's house, the doctor was the young sister of the family, so the leading female character must've just been lying when she told that part to the detective, so that we wouldn't suspect of her friend, I suppose... still, there's a lot of options about the reason about the male killings, I think it's up to us to decide? They could've known about the father's killing which I think is the best one, because they might've known about it as one of them had the room filled with cameras, and probably got to know about it. I also didn't quite understand why the doctor was going to kill the leading character? Jealous? Regret?

OK, this is what I believe happened, and I am quite certain this is right, or at least the most accurtae of the interpretations on here, I hope this clears some things up:

The doctor was living next door to her when she was young and being raped/molested by her father, whether she wanted to be a boy at the time, the main gilr was lying etc. is irrelevent in the rest of the story, what is important is only that you know the cop was made to believe it was a boy that lived there, and that he died in the fire. They became lovers (more or less)and would entice men, and then brutally murder them, as a way to get back at the male gender due to the horrible way she was treated by her father. I think the first murder was her father, thats why they had the head in the freezer of the "work room" and the body being preserved in that house. The body parts were what linked everything, especially the victims together. Someone said earlier on that the drawings of his head was meant to "explicitly symbolize" that he was going to get his head cut off, i think this is really far off base. I think it was just a matter of admiration, he was the only man that seemed to show genuine care/concern for her, he trusted her, which in the end is what saved him. The little boy who was killed, was just a way to get detective Oh to the crime scene as well as tie other things together, there was no major relevance in it though. The things that cause most confusion seems to be what thell her doctor/friend was doing with the blood, and why she went to the record store. I think what it was, was that she was intentionally going to take the fall for her lover, the blood was a victims, it was neither hers, nor the main girls. So she was going to go to the record store and make it look like it was her and she was going to pretend to try to kill her friend, because her love and care for her was so strong.

I have a question though... what about the head on the male suspect's freezer? The guy that was always after her... wouldn't it mean he knew about it all along?

No No No No No. The doctor you keep talking about...with blood...at the record store.....is her brother. The one we see getting burned earlier in the movie. Don't you recall the scars?

She didn't have a brother. The doctor friend was her neighbor.

It was but the ending threw me off! I have no idea how he found out about the headless body in the tank or anything else in the end for that matter....I'm so confused..
By the way did anyone notice the head in the detective's car? The first two flashes we see there's no blood but the third time it's covered with it...strange

The detective found out about the body in the tank as he was inspecting the photo Detective Oh left him to discover. The tank was behind the two men on the left.

I just noticed the thing about the scars...I didn't really get that part, I was confused until I saw this thread...but I missed the part in the movie about boy turned girl thing. Meh, still a good movie, makes you think.

The girl doctor is the 'young boy' the main actress had mentioned her as a boy in her past, because they hardly know each other, and only through a window. Even after the fire, she doesn't know it was a girl since her short hair made her like a boy.

the girl survived and have developed affections on the main actress, and do the butcher job for her .. whether the main actress had known she was the boy or she wasn't, is what the director leave to audience to decide.

She is the neighbor. Remember the staffer telling Dr. Cho that the neighbor had six daughters, no son, and the youngest daughter was named Sarah (the friend doctor's name).

I agree with most of this. But not about the boy. If you pause the DVD and carefully look, the boy peeps over the edge of the window and sees Susan, who is at the carving table. So the boy has to be killed, since he might've noticed or understood what he just saw...Susan in the midst of a killing, or at least in attendance (we don't know if anyone else is there, out of sight).
