James is not schizophrenic. he may be mentally damaged or have trauma and PTSD from his experiences, but the Bob voice does not mean he is schozophrenic. It can be a lot of things. I interpret the voice as memories or confusion from the "prison" or even in his head, but in all honesty, he comes out to be more sane than most people who have witnessed what he has.
