When Rachel is giving birth to Emma, the doctor suddenly notices that the baby is emerging butt-first. Whuh? All those checkups and they never noticed???

Even back before ultrasound, doctors could tell if the baby was settling into a breech position. My brother was that way, so the doctor had Mom go to the hospital a few weeks beforehand and they were able to reposition him, apparently by just pushing on her belly.

Breech birth is extremely risky, so these days they generally plan a caesarean. In any case, you DON'T just tell the mother to "push harder"!
