Great story and great misdirection! (SPOILERS!)

I love the way DePalma thwarts our suspicions that Michael Caine is the blond killer in drag by showing him getting taunting messages from his transexual patient "Bobbi," which persuades us Caine CAN'T be the killer.

It is only later that we realize we've been played.

Brilliant.

I thought it was pretty obvious from the minute he didn’t tell the cops about the message he got after the murder and then I could tell it was him even with him in a wig, sunglasses and make up

That was a good move. A great twist should make sense of what we’ve seen earlier, and this one really does.

I flitted between suspecting Caine and then disregarding him. I knew the blonde was a dude immediately, and it looked a bit like Caine but the mouth seemed slightly different. I actually thought it might be the other psychiatrist (the mayor from Ghostbusters) because of his thin lips and slightly camp manner.

I mean it had to be Caine really because there just weren’t many major adult male characters, but the film was able to throw me off the scent a few times and that’s good enough for me.
