Why was the woman screaming at the end of the movie

She'd been driven mad by the torture and murder of her beloved uncle, her rape and torture at the hands of Hopkins and Stearne and seeing her betrothed, Richard, going insane and hacking Hopkins to death with an axe. I think that pretty much explains the screaming.

The idea that Sara was "driven mad" and Richard had "gone insane" is an extreme interpretation. The ending depicts Richard mad with rage hacking Hopkins (Price) to death while his just-tortured fiancé screams and screams. The evil inflicted upon them has brought them to this point of maniacal frenzy, but it was a temporary coping mechanism rather than a permanent condition. People often morph into screaming madmen or madwomen during a serious fight (if for no other reason than to get their adrenalin pumping and intimidate their foe), but they're otherwise perfectly sane.

Despite the rather downbeat ending I've always viewed it as somehow uplifting for obvious reasons -- the bastage got exactly what he deserved and justice had been served. There's no reason we shouldn't assume that Richard & Sara moved on to live a happy life together. If anything, the horrific experience made their link even closer, like the unbreakable bond of buddies who survived the life-or-death battles of war together.
