First of all I know the Sergio Leone spaghetti westerns were technically Italian productions, but I do not think of them as Italian films, on at least a few levels. Their subjects concerned the American West, not even Italian characters in the West - not Italian subject matter. The lead actors were mostly not Italian, even if the rest of the casts were. (Lemme see - Gian Maria Volante was Italian. Even Claudia Cardinale, although she considered herself French as well as Italian) And they were shot in English.
For comparison, do people consider Blow-up by Michelangelo Antonioni an Italian film? I certainly don't.
So, given that explanation, my list is as follows:

Fifth is a tie between La Notte and Juliette of the Spirits.
Also should mention Nights of Cabiria, Red Desert, Amarcord, The Conformist, Two Women (with Sophia Loren) and Fellini Roma.
I should mention i have never seen The Leopard, but I intend to. People rate it highly. I also should mention I do not think Wertmuller's films have held up, altough I liked them at the time. And most of DeSica is largely lost on me, although I did mention Two Women and might want to see The Garden of the Finzi-Continis again to see if it is worth mentioning.
