I noticed them too and I thought they were there to convey some meaning, especially given the constant comparison between Elio and apricots (ripening fruits etc). For ex, in the end, when he's crying, we're in the middle of the winter and it's odd that there's a fly in his sweater, so I thought it was on purpose. In this case, to indicate he had "matured"/ripened after the whole experience.
But maybe I'm reading way too much into this and these were just random flies.
