Tom was a ghost, right? He had to be. There was shown no footage of her researching who it was that gave her her heart. Plus, how would she know about the phone in the cupboard?

However, wikipedia and another source I read say it's her imagination with Tom. But wouldn't we need more proof for that assertion?
