They were going to lobotomize him, but Glass sabotaged the equipment when he escaped his room so it didn't work. He killed the intern with the piece that he removed from the laser (or did something evil with it, I can't remember now). It was kind of silly, there would have been some sort of warning or indication that the equipment wasn't working.

Being the genius that he is, Elijah must’ve also found a way to ensure this wouldn’t happen. His plan was remarkably well thought out. Unrealistic? I wouldn’t underestimate what Mr. Glass is capable of, that’s all I’ll say.
By the way, Elijah actually used a piece/shards of glass from one of his own picture frames to kill Daryl the orderly (Darly noticed and mentioned that the frames were broken/crooked). share
they tried to lobotomize him with psychiatric medicine. this isnt realistic tho. you can be on the medicines for 20-30 years with no effect on you. so realistic it isnt.
