Did I miss something?

In the final episode of Season 1, Dr. Chance's knife is at the hospital with his belongings. Yet, we are told (shown) that D pulled a knife out of Blackstone's chest, which killed him. This was supposed to be Dr. Chance's knife because the doctor presumably stabbed Blackstone with it.

If that is all correct, how did the doctor's knife get from the motel room and back into the doctor's pants pocket in the hospital? I've looked online for an explanation, but I cannot find one.

I think it was just the empty sheath. I don't remember seeing the knife in it.

It was the pocket sheath that was on him, the knife was still in Blackstone's heart.
