If you have a double mastectomy, then any massive tatas you have afterwards are going to be implants. The big difference between hers and the ridiculous looking porn actresses with implants is that she had the money to get them done properly and has enough money to get the follow up surgeries that boob jobs need if the are to look decent years down the road. Sadly most women that get implant either had doctors that never explained the need for follow ups to adjust them as the woman ages or didn't understand it when the doctor told them, because even the most natural looking implants will start to look bad after about 8 to 10 years time, some sooner but most all by 10 years will need follow up. It's why you see so many bad looking boob jobs at the beach on older women.

From watching porn, the usual way to do implants is the underboob slit cut where it's only noticeable once they're lifted. The size of the cut varies from the ones I've seen with longer scar lines to just a tiny hole. I'm assuming one method is pre-filled and the other is just an empty implant stuffed inside the boob first before being filled to leave as minimal a scar as possible.
A newer method that I have noticed recently is that they cut out half the areola instead and stuff in the implant that way before sewing it up which looks even more natural due to part where the darker skin pigment meets the lighter skin from the rest of the body which blends the scar really well.
As for older women, don't most older women have bad looking boobs as they age? Sagging and such. I've not seen a bad boob job as one gets older. Got any examples?

Go to any crowded beach or pool in the summer and you can usually see some of the bad jobs.. but as I said they aren't bad because they were bad to begin with they are bad because they haven't had the touch up work that they require as the woman ages, her body changes and gravity has worked to stretch the skin.
I'm well aware of the different methods, the problem Jolie and any woman with a mastectomy have is with the nipple and areola that is normally removed. Even if they have a nipple-sparing mastectomy the nerve endings that connect to the nipple are severed so the sensation is gone. So even when they have an elective mastectomy because of the expectation of breast cancer in the future it doesn't matter if they have the nipple spared it will still lose any sensation. It also is unlikely that someone seeking an elective mastectomy would have the nipple sparing version as it the women with that gene predisposing them to breast cancer are still likely to have cancer start in the nipple. Most likely eveything was removed along with some skin as it isn't like they would just open up her chest and remove the breast tissue inside and replace it with saline or silicon.
As for locations you left off the transaxillary or armpit location.
