I don't think the big bearded guy did it. I think the fat bearded annoying dude went all Vincent Van Gogh on himself out of enraged insanity after his wife admitted cheating on his juvenile ass. Weird twist, I suppose.
No idea what the movie's title is supposed to evoke. Maybe they just got lazy with trying to think of one?

I agree that the final ear wound was self-inflicted. That box had been gifted to him, and he passed it on in kind.
About the title, given all of the historical references throughout the movie I suspect the allusion is to the apple in the garden of eden. Remember that the apple represented forbidden knowledge and caused a fall from grace when known. Just like the characters in the movie when they learned things that probably should have stayed unknown.
