What was the deal with the maitre-d at her apartment saying she had packages and he would send them up?
Later, when she gave him a tip, it seemed to imply that he had been in on it and sent the guy up knowingly.
Was that what we're supposed to believe? And then why the hell would she tip him at the end?

The implication is that the concierge was either paid by the goomba or threatened by him in order to gain access to Molly's apartment. More than likely, he was simply paid a c-note or something like that for looking the other way and for telling her that she should expect packages to arrive upstairs.
She tipped him as an admonishment, or rather as a method of shaming, essentially telling him:
I know you gave them access and THIS (the tip) is all you're good for.
