When Thanos agreed to "spare" him, did I misunderstand something and that included healing him? or does his suit have some kind of nanotech I've forgotten?
The, uh, Iron Man Sword (?) went through Tony's nanotech armour but not his impenetrable plot armour.

Not an unusual thing to happen. Bullet wounds are quit similar with more lethal rounds being produced / used that break up and or cause a larger exit to cause max damage.
In Tony’s voice: “It’s Nanotech.”
That about covers it. The blade went in all the way through, but like previously stated... no major organs were damaged, thus Tony was able to heal himself with his glorious Nanotech. The science behind it... that’s for you to figure out.
It appeared that Tony got stabbed in a vital organ, the liver. However it's possible that Thanos missed his liver and that Tony's nano-tech suit had some kind of medical nano-tech that sealed the wound and treated any internal injuries.
Remember that this is post Black Panther, so the world knows of Wakanda. Someone with Tony's resources could likely get his hands on Wakanda's nano-technology and medical technology.
Tony wouldn't need Wakanda as he has his own nanotech but yes, it was nanotech.

It seems like in movies, if the person is stabbed in the side instead of directly in the middle, they survive.
