Still don't understand (spoiler)

Who killed the little girl whose parents went to see the medium in the prologue? If the medium didn't kill her, why was she haunting the house all through the main portion of the movie? Was it her dad who killed her, the medium, or someone else entirely? Also, who possessed the medium?
