My theory

Everything is real up until the crash. Neil dies and Lily goes into a coma. Lily then imagines what her life would have been if Neil never existed. She finally realizes she loves Neil and should have agreed to marry him. She no longer feels guilty about rejecting his proposal and is able to let go and wake up from the coma. When she wakes up she starts crying because she knows Neil is dead before anyone even tells her.
