Early on he was coughing into a handkerchief; it wasn't as noticeable. I *think* that he could continue to live, even coughing blood, as long as his organs continued to function. As I understand it, TB was known as "consumption" because the disease would slowly consume the whole body, not just the lungs. As much as he was coughing and spraying blood into the air, I'm surprised no one else caught it, especially his wife.
