Are the actual nurses actual nuns. It seems like it but then it would be much more of a shock for the evil one (is it Quayle) to find out that Joan had a fiance.

The shock of Joan having a fiancé is that the professional nursing organizations required that the nurses be unmarried.
I actually saw something that answered this question a few weeks back...it talked about the tradition of nuns as nurses in the Empire and that even after the nurses stopped being actual nuns they were still called sister and still wore nun-like head gear. Thanks for the additional information!
