Am I the only one who spends half of almost every episode trying to figure out if the reenactments are the real people or just actors who really really look like them?

I agree. Whoever does the casting for this show should go on to bigger things too. They sure are spot on. I can't tell most of the time they are so good. I have seen big time movies that do not have great casting like this.

Actually I have a friend who has worked on this show, and it is all about hair, makeup and wardrobe. They have a really great team who spends hours applying wigs, fake facial hair, extensions, fake teeth, contacts, etc. to the actors to make them look exactly like the real people. They even try to find clothes that match what the real people wore.

They have a disclaimer at the start of each episode that says some of the patients and actual doctors portray themselves in the reenactments.

If you google Bob Slay, ER doctor, you'll see he's real.
