Pretty sure she was in the hospital having an abortion. The events of the movie were symbolic of either her guilt for going through with or apprehension to go through with it

so in the state you live in, when they perform an abortion they slice you open? maybe she didn't have a good hmo

I was in agreement with the OP. WTF.

We knew from the beginning it was a delusion. But what we were supposed to take away I am not sure.

I think she had the baby and was looking at him or her at the end. But why were her arms all cut up?

Did she try to do harm to herself?

It was symbolic - not reality. Was all a dream after having a C-section. The kids Halloween bag symbolized she should have given boyfriend head - instead of getting pregnant. The rest was showing how raising kids is like.

She wished for an abortion on Hallows Eve, during a blood moon. This was explained, especially with the Robert Patrick character and the Samheim pagan symbols on the wall. It caused evil 'demons' to arise, to give her her wish, but unfortunately it would require her life.

The film suffered with its 'artsy' stylisation and heavy metaphor, possibly a result of the directors personal pro-life opinion.

I'm a little fuzzy on the ending. She has changed her mind, is looking lovingly in to the nursery....is this supposed to be 8 months later (as she was already 1 month along?). Is she to late and does the end signify that the creatures are taking her child? Besides a Caesarean I cant figure out why she had the cut/ operation on her stomach.

This is definitely a movie I'd be interested in reading as a novel to understand it better!
