I thought Jack was a girl and his mother just called him a boy to keep Old Nick away. He seemed to have no interest in the child and I thought it was because he thought it was a boy. I thought she was protecting her daughter from that rapist scum.
Jack seemed very much like a boy to me. Made more clear once he got the haircut.

Joy blamed Nick and when Jack came along wouldn't let Nick help, or touch or even see Jack. Nick deferred to this because Joy had been in so much grief over the first baby.

I thought the same as the OP for a while, And after the story started unfolding a bit i said to my self obviously he helped her delivering the child so of course he knew he was a boy.
