Be aware that this film plays with local characters, accents, politics and memes you will have to know ahead of time to understand the gags. It mainly relies on how the people from the basque country is seen by the rest of spaniards.

People in the Basque region probably speak diffrently too, right? With their own language etc, like the Catalonians who are differnt from the Catalans etc.

This is so difficult, because to understand some gags you need know a lot about spanish culture.
In general the basque are: very rude people, eat and eat a lot, are very traditionalist about their own language (the euskera) that it is completelly different to spanish and french and the other europena language because it has diferrent roots. Basque are very hard workers and they live practically detached to Spain, in fact the have a log long fight agains Spain about their freddom in a lot of cases with a long hard violence tradition.
In the other side the Andaluces (the boy) are very lazy people, they love love, dance and sing all the time in general the enjoy the life without concern.
Baques and Andaluce hate obviously.
But all of this are only stereotypes that was made about the comedy.
I feel this movie is only for spanish or people that know about spanish culture if not you need somebody that explain you every gags.

Guys you know and realize that is an exageration right? The Basque are not rude, and the Andaluces are not lazy.etc...is a funny exageration of the people that lives and the things other spaniard people said, but with a lot of true.

As a Spaniard it does certainly help to know a couple of the in jokes but I think most people can relate it to their own country. Every country has different areas that banter with each other.
I.e. you have the Northerners vs Southers banter in the UK.
The West vs East Coast beef in the US.
Every country is relatable.
As for stereotype I think it's Spain as a nation that is considered lazy, we don't particularly consider the South any lazier than the North. Especially with the recession whwre thoses in jobs are working their arses off.

Empecé a ver la película, pero mi mayor problema es que no los entiendo cuando hablan. Entendí el chiste de la gonorrea por ejemplo, pero la mitad de lo que decían no lo entendía por el acento ni por las palabras ni por lo rápido y pegado que hablan, así que dejé de ver la peli.

Same north-south differences (or regional in general) we have here - pretty relatable.
I am Mexican, never been to Spain, and I enjoyed both "Ocho Apellidos Vascos" and "Ocho apellidos Catalanes". I really liked both, they are not the best movies ever, but they are quite enjoyable.The Spanish accents are funny for us too, I guess it is because we get most of the dialogue (indeed some gags or jokes are not that easy to get), but most of the time everything is fine.
I would even watch a third film with Rovira and Lago.
Better than the whole franchise of "Tres metros sobre el cielo". Yet I prefer weird indie films such as "Estocolmo".
