Glaring Goof

Anyone else notice in the episode 'Deception in D.C.', the narrator mistakenly refers to the victim by her daughter's name? The victim was named Antoinette, her daughter (whom was interviewed extensively during the episode) is named Monique. But at one point early-on (when they still thought it might be suicide), the narrator says "...But if Monique really killed herself..."

How did no one catch/fix this? I backed it up several times so I know that's what I heard but I was shocked such a glaring mistake was left in the final cut.
