I don't get it...(spoilers from beginning to end)

I thought the therapist and Basia were the ones in conflict throughout time, but then the therapist was cancer??? And then the mother was with her throughout time, but where was she? Why did the one scenario change with the brother that killed brother? I was really interested until everything started falling apart, but still an interesting take on reincarnation.
