...I still had a nagging suspicion it was all a dream, especially after that scene late in the film where Robert goes back on his med and confides to his wife that he's dreaming of killing her -- to which she responds by weeping.

It never crossed my mind. Even with the inclusion of meds and his dead family... It was just presented in such a straightforward way... Although we never leave the house....

Any other hints? I don't connect the wife weeping with him dreaming or hallucinating all events depicted.

I agree. Having read this post before watching the movie, it was in the back of my mind. There is the wife weeping, but also at one point he mumbles in his sleep; the same exact phrases he's saying in the scene before.

Would be interesting to hear if anyone else considered this!

In the end I also thought the whole movie was a delusion of his though I did think he killed his family. I think it all comes down to his mental illness. The feel of the movie is dreamlike or delusional including everything happening from his point of view in a limited universe (his house). He also only eats eggs which is unrealistic for anyone.

Near the end of the movie there are a number of radio reports about a man on trial for the murder of his family. At the end the report specifically says they expect a conviction. Assuming the reports are real he must be in jail and on trial while living in his fantasy world. Given the above comment though I could see that perhaps the deaths and trial are also part of the delusion which would explain why the radio report of the trial seems to me mixed with the announcements of his advancements.

The deaths could also be something he added to his delusion after hearing of an unrelated trial on the radio.
