Why doesn't the mother-of-the-bride have a bindi (dot on her forehead) at the wedding?

Indian tradition for women to wear bindi. Though now, not regularly followed by all but very common sight that is in India.

Women whose husband dies do not wear bindi.

I think you mean the mother of the groom doesn't have the dot. The bride's father isn't dead so her mother should have the dot.

Then why did Judi Dench wear a bindi at the wedding? She was a widow.

Take it from a modern Indian woman:

Currently, bindi-wearers brook no rules. It's just an ornamental decoration. It's no longer worn in a traditional way in Modern India.
