Spoilers,
So they stole a baby 12/years ago for his heart for their son who looks about 7? By the end of the movie the boy looks 14. The boy in the bed while obviously sick and atrophied still couldn't be older than the other boy. That completely negates the plot making any sense. You can't kidnap one child to save the life of one you haven't even had yet.
I was thinking that same thing. But maybe it had something to due with his sickness or something.

I was thinking that maybe the kidnapped boy was around 3 or 4 years (when kidnapped) just to make sense on the film.
So she had her son, realized she needed the organs of another boy and then kidnapped the boy.
The twist for me was that how would they hide their son in plain sight.
Another post suggested their son might had the same age but remained smaller because of the illness, but it doesn't make sense to me.
So to me they tried to add too many twists, making the film hard to understand or believe.
The acting was very good, but the plot not so good. Makes perfect sense to name this crazy psycopath mother as Katherine.
Yes, I had some difficulties with that as well. And/but it was pretty obvious which boy had been ripped off from the hospital, judging by how each was treated.
