How come she raped him when he was a baby? Harvesting organs for an unborn child? The age difference between them seems very obvious. :O

Maybe Andy was just suppose to be small for his age perhaps due to the fact he was either born ill or became ill at very young age. He could have just looked young despite his illness.

I agree with the original poster. The age difference was clearly huge - Andy (or Jason) was about 13 but the boy in the basement could be no more than 9 - so Jason was kidnapped years before Andy was born!
