We don't know. I found it odd that we were shown the young couple dead in their bed and the older couple in the park taking off their oxygen masks, which showed that they were just going to let the pathogen do it's thing, and that they would die, but this couple's fate was not shown. Why show 2 and not the 3rd? I'd love to know if RD chose to stay with JH or go outside and probably die. Would you choose to live in a world where there were very few other survivors, or would you choose to end it in a quick, painless way??
Scary choices either way.

I don't know about you but "a world where there were very few other survivors" sounds pretty good to me. A chance to start over? People not living stacked on top of each other because there's no more space on the ground, basically driven insane by the relentless proximity of so many others, and fighting over territory because there's just isn't enough space or food or water anymore? Imagine - you can hear the planet breathe again, you can see the stars...and you've got the opportunity to do it right? Sign me up.

Anyway, I think at the end, we saw the descendants of the survivors, living in a new, clean world, starting over. The little girl found the ring, which of course would last for millennia, still around, long after its owner was gone.

But what if you weren't one of the few chosen to start over with? Or even worse, if your entire family died, leaving you to carry on without your children, husband/wife, even your friends. Seems pretty drastic to me.
I did like the last scene with the mother and the little girl. Sweet and hopeful.

Or what if you were one of the lucky ones and got an infection. Now let's see, how do you make penicillin again? Is that the one I even want? Sounds pretty sucky to me.

I kind of thought it didn't matter either way they most likely died. Their relationship was pretty emotional and volatile from what I gathered they weren't a good match. Even in what were probably their final days of life they couldn't agree and each getting their own way was more important than just being together in the end. Juxtaposed to the elderly couple who despite everything going on stay together until the end.
