My vote would be for the old wooden ironing board attached to some sort of metal wheel to create a coffee table. Not only was it ugly, it looked like a bump your shins magnet.

The small box/crate that the couple used 3 milk bottles as legs for was terrible.

The horrible lamp made with the carousel horse. Same episode: the toy car turned bar :P
