Incorrect. Plenty of people drink and smoke and stay in great shape (myself included). And you don't know what his diet was like. Just because they never showed Joe eating protein and working out doesn't mean he wasn't eating a steak everyday and pumping iron on a weight set in his backyard.

shareHe also works what is repeatedly referred to as an incredibly difficult physical job. If you do hard physical labour every day I suspect it would take more than drink and cigarettes to completely undo the effects. He's never depicted as a hopeless alcoholic, and the smoking does cause chronic coughing. I don't see anything too shocking in it.
I'm a lot more impressed that he didn't die of an infection after taping his gunshot wound closed. He seems to have gotten over it very quickly in fact.
