The girl would have realised that her mum was suddenly upset for some unexplained reason and she wouldn't give up till she knew the truth, which her mum would have eventually blurted out. Plus, her mum would have shared the information with others, who would have let slip. There's many ways this kind of thing can become known and so Valentin did exactly the right thing by keeping stumm. The doctor wasn't going to tell anyone, was he? Patient confidentiality and all that.

A lot of people believe that it's better to tell children the truth. Based on the fact, that she told Maggie the truth about the letters, even though she knew it would upset her, I could see why he would worry she would insist that Maggie should know. He couldn't take that risk.
